- We provide all the necessary paperwork to enroll patients in our Connected Care program to kick off their IAHealthQuest and all follow-up communications throughout the program.
- You will have 24×7 access to patient dashboards. Our Care Captain will coordinate with your office to schedule patient communications, integrate specialists as needed, and execute any changes to the patient care plan.
